Unclear levels. I get lost in games pretty easily so i hate it when i can't easily determine where to go next.
I also hate jumping puzzles and this is the main reason i tend to dislike platformers.

Like miyu, in an action game, or any game that is not a puzzle/adventure type game, if I can't find my way to the next area, I lose interest and turn it off.
Games that have cut scenes that are not skippable are also on my naughty list. If I want to watch your scenes, I will. Jerk.
I want to be able to save when I have to stop playing. Stupid developers.
If your game has sucky controls, I want to change them. Don't make me use your lame setup just because you got used to it during the 2 years you had to play test the dumb thing.
That should suffice for now. :)

Of the two i bolded i will completely agree with the second. My stupid console has a HDD - let me save anywhere i want and let me have multiple saves. If i want to fill 10 gigs with 800 save games while playing Crackdown let me.
On the first bolded point i want to also add that i want the ability to pause cut scenes. Sometimes someone calls, needs my help, etc. and i actually want to see the cut scene. Let me pause the sucker!

All entries sent in via email, twitter, facebook and the forum will be placed in a numbered list.
I'll use random.org to "draw" three numbers. Those people will win the prizes.
To make things easier, I'll probably number the prizes 1-3 and winners 1-3 will win the corresponding prize number.
